We are aware that most physicals, dental, and eye exams will be performed during the summer months. Schedule your appointments now to avoid the crowds closer to the start of school. All physical, dental, and eye exams must be turned into your school before the August 23 start date.

- Current Medical Physical
- Immunization Record
- Eye Exam
- Dental Exam
- 2nd Grade Students – Dental Exam
- 6th Grade Students – Dental Exam, Physical Exam, Immunizations updated
- 7th and 8th grade - Sports Physicals Due before the first day of tryouts or practice of any sport.
