Student Registration - Residency Requirements
Annual school registration for the entire District 289 will is held August 5, 2010 from 10:00 - 6:00 in the Northbrook School Gymnasium. Look for this important date in our Spring newsletters or on this website. If a family moves in during the school year, they should report to the grade level attendance center to register each child. Our District has 3 attendance centers as follows:
Blackstone School - 1309 Jefferson Street - Kindergarten and First Grades
Lincoln School - 805 4th Avenue - Second, Third, and Fourth Grades
Northbrook School - 1309 Guiles Avenue - Fifth - Eighth Grades and Preschools

In order to register a student parents must bring accurate address, phone, and emergency contact information to correctly complete registration forms. Students new to the District must bring proof of residency (utility bill, etc) along with a copy of the student's certified (not hospital) birth certificate.
Proof of Residency in the District Required
Illinois statute requires public school students to be residents of the districts they attend. Due to the number of families who wish to enroll their children in Mendota District 289, this law is stringently followed in the district. To enroll a child in Mendota District 289, permanent residency of the custodial parent or legal guardian, as well as the child, is absolutely required. All custodial parents and legal guardians are REQUIRED to provide TWO FORMS of evidence of CURRENT RESIDENCY within the boundaries of the school district, as provided on the landowners property tax statement. There are no exceptions to this very important issue. If a parent would choose to pay tuition into the District, that amount is variable annually but is the same amount as the cost per pupil for the district based on the previous year's audit.
Allowable Forms of Identification and Proof of Residency
Proof of residency must include a driver's license or other state-issued identification, as well as a second, recent form of identification with your name and address. This may include a recently dated rent receipt or current utility bill. The district may need to verify these documents or require a notarized Affidavit of Residency if necessary. Registration will not be allowed without absolute proof of residence, regardless of past attendance in the the district.
If it is found that a child is illegally registered in the district, that child will immediately be removed from district enrollment and other legal means may be pursued. Physical and Immunization Requirements
According to Illinois State law, a physical examination with completed immunizations is required of all PreKindergarten, Kindergarten students, new students, and 6th grade students before the first day of school in August. A lead screening is necessary for students below the age of 6. Dental exams are required for all students entering Kindergarten, second, and 6th grades. Vision exams are required for all incoming Kindergarten students and all new students entering school for the first time in the State of Illinois.
